The Internet says:

You can fix sagging with a bigger implant.

The Reality:

An implant is a weight, not a crane. Putting a heavy object in sagging skin just makes it sag faster.

Patients walk into my office every day with a specific request: I want to be perky again, but I don't want the lift scar. Can't we just fill it out?

The answer is almost always no.

If your breast tissue has dropped due to pregnancy, weight loss, or genetics, adding volume won't fix the position. It might look okay for six months, but gravity always wins. A heavy implant in loose skin eventually creates the snoopy nose deformity—where the implant sits high, but the breast tissue slides off it and droops low.

To actually fix the architecture of the breast, we have to separate Lift (position) from Augmentation (volume). We don't rely on silicone to do the work of a scalpel.

A breast lift is a surgical procedure that removes excess skin, tightens the breast tissue, and repositions the nipple-areola complex to a higher, more youthful height.

We grade the need for a lift based on ptosis (sagging).

  • Grade 2: Your nipple is below the fold.

The Incisions: The Tailoring Analogy

You Can’t Reshape Without Seams

The most common fear is the scar. But think of a breast lift like tailoring a dress. You cannot turn a size 10 dress into a size 2 without creating seams. The more skin we need to remove, the more seams (incisions) we need to make the shape look natural.

1. Periareolar(Donut Lift)

This incision goes only around the areola. It is reserved for very mild sagging. In my experience, this rarely works well for the post-partum mom because it tends to flatten the breast rather than lift it.

2. Vertical(Lollipop Lift)

The workhorse incision. It goes around the areola and vertically down to the crease. This allows us to lift the nipple and tighten the bottom pole of the breast, creating a nice, round shape without a horizontal scar.

3. Inverted-T(Anchor Lift)

The gold standard for significant droop or massive weight loss. It includes the vertical scar plus a horizontal scar along the breast crease. It allows for the most dramatic reshaping and is often necessary to remove the maximum amount of sagging breast tissue.

Here is the problem with a standard lift: your tissues are weak. That is why they sagged in the first place. If we rely on your own compromised skin to hold the weight of the new breast, it can eventually stretch out again.

This is where the Internal Bra comes in.

We use a bio-absorbable mesh scaffold (like Galaflex) that acts as a hammock inside the breast. It supports the breast tissue and holds it in that high, perky position, taking the tension off the skin. Over time, your body absorbs the mesh and replaces it with your own collagen and scar tissue, creating a permanent internal support structure.

Recovery is a psychological game. When you wake up, your breasts will look high and tight. They might even look square or boxy.

Do not panic. This is normal.

Because we have tightened the skin and the muscle (if using implants), the breast sits high on the chest. Over the next 3 to 6 months, the muscle relaxes and the tissue settles. This process is called Drop and Fluff. The breast will gradually round out and settle into a natural curve.

Scar Care:

Scars are red and angry for the first few months. We will start you on a protocol of silicone tape or gel around week 3. By month 12, most scars fade to a thin white line that is barely visible.

Will I lose nipple sensation?

The risk is low, but present. We preserve the nerves whenever possible. Most patients report temporary hypersensitivity or numbness that resolves as the nerves wake up over 3-6 months.

Can I breastfeed after a lift?

Usually, yes. We typically use techniques that leave the milk ducts intact, connected to the nipple. However, we cannot guarantee milk production, especially if you had supply issues before.

What is bottoming out?

This happens when the implant is too heavy for the skin, and it slides below the nipple, making the nipple look too high. We prevent this by using the Internal Bra (Galaflex) to support the implant weight.

Can I get a lift without implants?
